- Ask us to correct information you believe is incorrect or incomplete.
- Request confidential communication and ask us to limit certain uses or disclosures.
- Request a list of certain disclosures of your information.
- Get a paper copy of the current notice.
- Choose someone legally authorized to act for you.
- File a complaint if you believe your privacy rights were violated.
Your choices
In certain situations, you may tell us how you want information shared, including with family or friends involved in your care. Uses that require written authorization will not be made without it, and you may revoke an authorization in writing as permitted by law.
How information may be used
Health information may be used or shared for treatment, payment, and health care operations, and when otherwise permitted or required by law. Additional protections may apply to particular types of records.
Our responsibilities
- We are required by law to maintain the privacy and security of protected health information.
- We will notify affected individuals when a reportable breach may have compromised their information.
- We must follow the duties and privacy practices described in the current notice.
- We will not retaliate against you for filing a privacy complaint.
