When you request an appointment, you may provide your name, phone number, email address, preferred day, the reason for your visit, and an optional scheduling note. Please do not include diagnoses, symptoms, medical records, insurance identification numbers, or other sensitive medical details in the website form.
How we use website information
We use the information you submit to respond to your request, contact you about scheduling, operate the website, protect it from abuse, and meet applicable legal obligations. We do not sell information submitted through the appointment form.
Email and website forms
Ordinary website forms and email are not substitutes for the patient portal and should not be used for urgent or confidential medical communication. Existing patients should use the secure patient portal or call the office. Call 911 for a medical emergency.
Service providers and security
Hosting, email, security, and other technical providers may process limited information on our behalf when needed to operate the website. We use reasonable safeguards, but no internet transmission or storage system can be guaranteed completely secure.
Retention and your choices
Website inquiries are retained only as long as reasonably needed to respond, document the request, protect the site, or comply with law. You may ask a website privacy question or request correction of contact information by contacting the office.
